The following four (4) releases are required to reactivate your member account. They must be completed BEFORE returning to the pool.

      1) General Consent and Release Agreement
      2) General Code of Conduct
      3) Wellesley Sports Center Code of Conduct
      4) Swimming Safely COVID-19 Protocol

In addition, swimmers will be asked their membership preference for reactivation (i.e., monthly, 3-month, 4-month, etc.). Explanation of cost, payment/renewal, and credit usage will be described to help members make their selection.

All items with a red asterisk are required - please read, e-sign, date, and initial when appropriate. Thank you!
