To officially reserve your retreat spots, complete this form & submit a $50 deposit to the office.
No matter which retreat(s) you register for, the total deposit is $50 per teen.
FORMS & DEPOSITS DUE March 14 by 11:59 p.m.

Payment (cash or check) can be turned in to the parish office. Checks can be made out to Assumption Life Teen.

COVID Policy: Your safety is our priority. While we recognize that no one can tell us what June & July 2021 are going to definitively look like, we do know that our Summer Retreats will look and operate differently than they have in the past. A full list of our Summer Retreat COVID policies will be distributed by email and will be subject to change based on the most recent guidelines at the time of the retreats. We will be releasing new information as it becomes available.
