Virtual Referral Form
As a parent, relative, student, teacher, or stakeholder, you may refer a student for the Farmington Gifted and Talented Program. Completing this form will allow our GT Coordinator and Facilitators to send home the appropriate paperwork for the student that is being referred. Parent permission is required for any screening to occur, but anyone can make the first referral to begin the process.

In case of referral by an individual outside the traditional family structure of the student being referred, please provide an address to which the appropriate forms should be sent. The student and their guardians will have their own opportunity to fill out these forms as well, and their permission is required for any sort of screening to take place, but all stakeholders can complete this exploratory part of the identification process. If no address is provided here all paperwork will be sent home with the student or mailed to the home address on file.
