Chromebook Acceptance Form 2020-2021
Students in the McGuffey School District will be issued a Chromebook for their educational use.  This Student Acceptance Form ensures that equipment supplied by the McGuffey School District will be used and maintained in its original condition, reasonable wear and tear excepted, by the student who signs it.  By completing this form, the student agrees that he/she will demonstrate reasonable care with the respect to the security and physical well-being of the equipment assigned to his/her care.  The student will be financially responsible for any damage that exceeds reasonable wear and tear as well as lost or stolen devices.  McGuffey School District has the sole discretion to determine the condition of the equipment and whether or not the student demonstrated reasonable care in the use of the equipment.  The Chromebook and its accessories are the property of the McGuffey School District.

Please do not remove any Identification Tags from any component.  If in the event that any of the tags become removed, please notify the school office immediately for a replacement tag.

By signing this agreement, the student acknowledges and agrees:
To abide by the McGuffey School District Acceptable Use Policy (https://tinyurl.com/MSDAUP ).
To use the Chromebook for educational use.
The only authorized users of the Chromebook are the student and his/her parents or guardians.
That while the equipment is connected to the Internet, its use will be logged.
To not attempt to circumvent the District’s Internet Content Filtering.
That the remote Internet content filtering provided by the district is provided as a convenience only.  The filter should not be used as a substitute for parental supervision.
To report any problems or damages immediately to their teacher.
That the device and its accessory equipment (Case, Battery, and Power Cord) are the property of McGuffey School District.
To return the equipment before the last day of school if the student is transferring out of the District.
That if the equipment is not returned to the District in its original condition, reasonable wear and tear excepted, or if the equipment is damaged, lost, or stolen, due to Student’s negligence or not utilizing reasonable care in the use of the equipment, the student agrees to be financially responsible for the replacement value of the equipment as determined by the District.
That Accidental Damage Insurance is available for purchase and that if the insurance is not purchased, the student is responsible for the total cost of any damage that occurs to the Chromebook and its accessories.
That all information stored on equipment carries no expectation of privacy and is property of the District.
That it is the Student’s responsibility to charge the device each night and bring it to school each day.

By completing this form online, the Student and the Parent/Guardian acknowledge and agree that any information or use of the equipment carries no expectation of privacy.  The District reserves the right, at all times and without prior notice, to inspect and search and an all its property for the purpose of determining whether any policy has been violated, or when an inspection and investigation is necessary for purposes of promoting safety or compliance with state and federal laws.
