Due to the 2019-2020 outbreak of Coronavirus (COVID-19), INTENSIFIT Ltd is taking extra precautions with the care of every gym and staff member.

The main symptoms of coronavirus (COVID-19) are a high temperature and continuous cough. If you have any of these symptoms or are feeling slightly unwell please do not come to the gym or take part in any indoor or outdoor sessions, this is to protect our staff and other members, please Stay at home and call 111.

The member understands the symptoms and affirm that they, as well as all household members, do not currently have, nor have experienced the symptoms listed above WITHIN THE LAST 14 DAYS.

1. I affirm that I, as well as all household members, have not been diagnosed with COVID-19 WITHIN THE PAST 30 DAYS.

2. I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 WITHIN THE PAST 30 DAYS.

3. I affirm that I, as well as all household members, have not traveled outside of the country, or to any city considered to be a "hot spot" for COVID-19 infections WITHIN THE PAST 30 DAYS.
 
4. I understand that INTENSIFIT Ltd cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each client.

The member acknowledges, appreciates, and agrees that:

1. Participation includes possible exposure to COVID-19. While particular rules and personal discipline may reduce this risk, the risk does still exist; and,

2. Knowingly and freely acknowledges all risks when entering the gym or taking part in outdoor sessions,  both known and unknown, even if arising from the negligence of others, and assumes full responsibility for their own participation; and,

3. Willingly agrees to comply with the customary gym and COVID rules to reduce risk. If, however, the member observes any unusual or significant risks during participation to themselves or others, they will remove themselves from participation and/or bring such to the attention of the nearest INTENSIFIT staff member immediately; and,

4. I, the member, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS INTENSIFIT Ltd their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the business (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
