Please use this form to let the directors know of UHS Band absences. (In-school absences will be handled through the regular Mon County Schools system and need not be entered here.)

You do not need to fill out this form for late arrivals or early departures, though students should inform a director before arriving late or leaving early.

Please remember that not all absences are automatically excused. Doctors notes are always excused, as are things such as death in the family, medical emergencies, etc.

Summer camps, family vacations, school trips, and parental custody issues are generally always excused.

Examples of things that are generally NOT excused include, but are not limited to, student work conflicts, failure to arrange for a ride, birthday parties, etc.

We review each absence on a case-by-case basis.
