PERFORMANCE Registration Form
Vietnamese American Services (VAS) is a non-profit organization established to address new and emerging needs of the Vietnamese American population in Maryland. VAS is committed to bringing the best and most convenient services to the community in order to link members with resources and improve their quality of life.

This year, VAS will hold the Mid-Autumn Festival 2022 on:
Date: Sunday September 11, 2022
Time: 1pm - 4pm
Location: VAS Office Parking Lot (12621 Old Columbia Pike, Silver Spring MD 20904)

Although the festival begins at 1, performers are expected to be present at 12:00pm in order to do a sound check before the performance.

If you are interested in Performing at our Mid-Autumn Festival 2022 Event, please fill out this application. Please note that this does not guarantee you a spot on our Program.
If you would like to perform more than once, please fill out a separate application for each performance.
Performance Registration Form closes  September 2, 2022  at 3:00pm EST.
If you are still interested in participating in our event but the application deadline has passed, please reach out to VAS staff at info@vasusa.org or aneesa@vasusa.org

Approved Performances will be contacted the first week of September
