Birthday Request Form
A 45 min Musical Me in OKC Celebration where we come to you! $140 paid 48 hours after birthday party confirmation. First 15 minutes is instrument exploration while we wait for all of the guests to arrive. The last 30 minutes is the themed birthday experience. We will bring different instruments, specialty props, and finish with the parachute! The parties are geared for 1-5 yr olds. We bring enough items for 12 party goers, so please let us know if you expect more than 12. Parent/Caregiver participation required for children 3 and under. Parent/Caregiver supervision required regardless of the age.

Party Location Address (Make sure you have chosen a place with ample space to make a circle with the party guests, dance, etc.  Open concept living rooms, club houses, library room rentals are perfect!) If your location isn’t within the OKC metro, we may require an additional fee to compensate for time/gas.  *
