The Mathematics Education Section (MES) of the Education Bureau (EDB) is committed to fully compliance with the relevant provisions of the Hong Kong Personal Data (Privacy) Ordinance in handling and keeping confidential the personal data you provide. The personal data collected in this form will be used for the following purposes: (1) Results announcement, including the names of participants and teacher supervisor, and their school names; (2) Any arrangements and internal analysis related to this Competition; (3) Disclosure to other government bureaux/departments for the promotion of the competition or other education initiatives by the EDB; (4) Other purposes in relation with this Competition. The provision of personal data by means of this form is obligatory. Failure to provide these data may affect your nomination. You have a right to request access to and correction of your personal data submitted as provided in the Personal Data (Privacy) Ordinance. Enquiries concerning the personal data collected by means of this form, including making of access and corrections, should be made in writing to Mathematics Education Section, Education Bureau (email: math@edb.gov.hk).

教育局數學教育組會依照《個人資料（私隱）條例》的有關條文處理及保密你提供的個人資料。從本表格所蒐集得到的資料只會用作以下用途：(1) 公布結果，包括參賽者及指導教師的姓名及所屬學校名稱；(2) 與比賽相關的一切安排及內部分析；(3) 向其他獲授權的政府決策局/部門披露以推廣比賽或教育局其他教育措施；(4) 與比賽有關的其他目的。你必須在本表格上填寫個人資料。如未能提供所需資料，可能影響提名資格。根據《個人資料(私隱)條例》，你有權要求查閱及改正你所遞交的個人資料。如欲查詢有關本表格所蒐集的個人資料，包括要求查閱及改正資料事宜，須以書面方式向教育局數學教育組提出（電郵：math@edb.gov.hk）。
