St Mark's Primary School Eisteddfod 2021 Individual Entry Form
Please ensure that you visit www.trybooking.com/BTUNQ to pay the entry fee before completing this form. Only $5 per entrant (please only pay once per child, even if they are playing in more than one item).

Students in Years 2 to 5 should perform one piece of their own choice.  

If your child learns two instruments, you may play one piece per instrument. Please fill out one entry form per item if your child learns two instruments.

Vocal entries are very welcome if your child has vocal lessons.

Please note, this is NOT for students in the Year 2 String Program. They will play in a concert in Term 4.

PLEASE BRING COPIES OF YOUR SHEET MUSIC AND PIANO ACCOMPANIMENT TO THE MUSIC DEPARTMENT OFFICE.  
YOU MUST HAND IN YOUR BACKING TRACK ON USB AT THE SAME TIME IF YOU ARE USING ONE.

You may supply your own accompanist, although Miss Leah Guelfi (Music Teacher) is available to accompany students for no additional cost. A short rehearsal will be organised in the week before the Eisteddfod.
