With this form you can enter information about each quilt you would like to register for the 2022 EAA Quilt Show. You will be asked for your name, phone number, and email address followed by the name of your quilt, its description or story, its size, who it was quilted by and the category of the quilt. You can copy your story and paste it here from another document if you choose. You will need to complete a new form for each quilt you are entering.

You will be sent a confirmation email with all of your responses for each quilt you enter. Please review it for accuracy and email Peg Hamblin with any corrections that need to be made. (peghamblin@gmail.com)
