Thank you for your interest in Horizon Academy's summer programming. This form will provide you the opportunity to submit your preferences for the Summer Academic Program or the Summer Tutoring programs. Please select the program for which you have an interest, and for tutoring, which days and location work best for you.

Summer Academic Program: June 3 - 28 / Entering Grades 1-6 / Monday-Friday 8:15 a.m. - 2:45 p.m. / $3,550
(We may consider rising 7th grade students).

Summer Tutoring Program: June 3 - July 19 / Entering Grades 1 - 12 / 1:1 sessions / 45-minutes / 
2-days a week $990 / 4-days a week $1,980
Please note: There will be no tutoring July 1-4.
