About the survey: There are 6 different surveys that should take you approximately 10 minutes to complete. The first group of questions should be answered by the child and the second group of questions should be answered by adult.
If you would like to sign up more than one of your children in the program, please submit one sign up and one survey per child (two children max). The information will stay confidential and will be used to support the development of more Taster Space programs.
We will contact you with enrollment information if your child is accepted into the program.
The Taster Space program is partnered with UT Health to study the impact of availability of fruits and vegetables on children's consumption. By participating in this program you will be asked to provide information about you and your fruit and vegetable purchasing habits and about your child's fruit and vegetable consumption. The purpose of this study is to assess the impact of the Taster Space program implemented by Desert Spoon Food Hub on increasing availability and consumption of fruits and vegetables among participating families. You are invited to take part in this study because you and your child (ren) or family are planning to participate or recently participated in the Taster Space program. This study will help us better understand how to improve the Taster Space program as well as evaluate the effectiveness of the program.
If you agree to participate, you and your child(ren) will be asked to participate in surveys related the Taster Space program and Veggie-meter readings as described below:
1. Each survey will take approximately 5 -10 minutes to complete.
2. Veggie-meter readings will be taken using a Veggie-meter device that uses LED light (like in a common flashlight, which is considered safe for skin exposure).
The procedures mentioned above are harmless to you and will take place at the beginning of study participation and at 9 weeks after the completion of the program. The risks to participating in this study are minimal or unlikely. You may not receive any benefits from participating in this study.
There are no costs to you and you will not be paid to take part in this study. You will not be personally identified in any reports or publications that may result from this study. Any personal information about you that is gathered during this study will remain confidential to the extent of the law.
You can refuse to answer any questions asked or written on any forms. Your participation in this study is voluntary. A decision not to take part in this study will not change the services available to you from the PI or study staff.
