Per the Erie County Department of Health, please submit this DAILY health screening for each of your children each day. Submit a different form for each child.

Parents who answer "YES" to any questions should keep their children at home. They should not bring them to school or have them get on a District school bus. Parents who mark yes for one of their children should keep them home and call their medical practitioner.

If any children tests positive for COVID-19, please call your child's school as soon as possible after being notified.
