This form must be filled in weekly and payment made on ParentPay by 12pm on a Thursday to secure your child's place in breakfast club the following week.

The cost of breakfast club is £1 per session and it will run from 8.15am. (Families entitled to Free School Meals will receive this service free of charge) At the end of the form, if required, there will be a link to ParentPay. Please only pay for Breakfast club for the following week.
