Things to remember:
1.  This form will be used to assign your disable seating tickets.
2.  By filling out this form, you are not receiving an extra tickets or better seats in the arena.
3.  Disabled Guest Tickets are assigned seats for those in wheelchairs, their caretakers, and guests with limited mobility. Disabled guest and caregiver may be seated in a different location from the rest of your party. Your disabled guest will have one caregiver seat with them.
4. In order to assign the appropriate seating arrangements, please provide as many details as you can. For example, guest has trouble with stairs. Should be guest can handle one or two steps or guest cannot handle any steps.
