Please complete all parts of this form if your child is starting our school in September 2020. Please be aware all information will be shared with School Health, Social Services and other agencies.  Certain information regarding your child for example their name, class and date of birth is shared to educational programmes like Dynamo Maths, Class Charts.  A more in depth list of which agencies and educational programmes, information is shared with, can be viewed on our website or a hard copy can be obtained from the school office.
