Dear Parents/Carers,
For the safety of all children at Avenue Primary School, it is important that we know as soon as possible who you want to collect your child each day, or whether you are happy for them to walk home alone.
Please be aware that the school expect children in the Nursery, Reception, Year 1, Year 2 and Year 3 to be collected by an adult known to the school. Children in these year groups will not be allowed to walk home alone.
If your child is in Years 4, 5 or 6 and you want them to walk home from school unaccompanied, you must complete this consent form below to give your child permission to do so.
If you wish your child to be collected by anyone who does not have parental responsibility (e.g friend, neighbour, older sibling, other relative) then it is important that you complete this consent form. Collection of a child by a sibling under the age of 16 years old is done at the parents discretion.
If you would like your child to meet you at the front gate or at another class, you are still required to complete this consent form.
This consent is carried forward into a new academic year and so you should let us know if circumstances change.
Please ensure you complete a separate form for each of your children.
Please note that if your child will be travelling home via public transport, you will be required to apply for a Zip Oyster photo-card in order to get free travel on bus, Tube, tram, DLR, London Overground and most National Rail services where pay as you go is accepted.
