Please complete the following form to notify the state office who will be volunteering to fulfill the Region V Leadership Conference volunteer requirement. Each chapter is required to provide one adult volunteer at the Region V Leadership Conference. Chapters that do not provide a volunteer will be charged a $150 fee. If a volunteer does not show up or fulfill their commitment, the chapter will be charged a $150 fee. Volunteer names are due December 1.
