Agreement and Understanding
By entering my name below and submitting this
form, I, a parent/guardian of the above listed minors, grant permission for
each child listed above to participate fully in any activity related to
Vacation Bible School at First Baptist Church Vidalia, GA. I understand
that my agreement carries with it the following:
- An authorization of any of the
adult leaders to obtain necessary medical attention and/or treatment for said
minor by a licensed physician or any emergency medical personnel.
- I knowingly release, absolve,
indemnify, hold harmless First Baptist Church of Vidalia, GA, any homes where
activities might occur and any adult leaders from all claims that might result
from any injury or death of minor.
- First Baptist Church of Vidalia,
GA cannot guarantee that you or family members will not become infected with
contagious diseases such as COVID-19. On behalf of yourself and your children,
you hereby release, covenant not to sue, discharge, and hold harmless First
Baptist Church of Vidalia, GA, its employees, agents, and representatives, of
and from all liabilities, claims, actions, damages, costs, or expenses of any
kind arising out of or relating to your participation in our programs, services,
or activities. You understand and agree that this release includes any claims
based on the actions, omissions, or negligence of First Baptist Church of
Vidalia, GA, its employees, agents, and representatives, whether an infection
such as COVID-19 occurs before, during, or after participation in any such
program, service, or activity.
- Photographs are sometimes taken
during the activities the students will be participating in for publicity and
promotional purposes, which include, but are not limited to, in-house
presentations, social media platforms, church web sites, brochures, and
newsletters. By signing this area, you are releasing First Baptist to use
of photographs of the student as stated above.
- Should medical help be needed, I
agree to pay either directly or through my own personal health and accident
insurance policy all medical or hospital costs.
- I understand that I can rescind
this release any time with a written request to First Baptist Church of
Vidalia, GA. It is also my responsibility to let First Baptist Church of
Vidalia, GA know of any changes in the information I have provided.
