2019 Laurelville High School Retreat UYM Parental Consent Form
Union Church’s high school retreat is coming soon!  It’s an awesome chance to get away from the stresses of your crazy schedule.  You’ll strengthen friendships and build new ones.  And you’ll get a chance to get away from the noise and spend time with God.

Total cost is $150 (checks can be made out to Union Church). $50 deposit and completed consent form are due by October 27. Final payments and completed Laurelville Consent form (get from Kellyn or webpage) are due November 15.

Departure: Friday at 5pm from Union Church parking lot
Return: Sunday at 3pm at Union Church parking lot

Packing List:
Warm Clothes (jacket, hat, gloves, old shoes/boots, etc)
Mud Clothes (2 layers of stuff you don’t mind throwing away, think thrift store prom dress/3 piece suit)
Bedding (sleeping bag and pillow)
Bible
Toiletries (towel, tooth brush, toothpaste, comb, soap, shampoo, deodorant, etc.)
Extra $ (meals to and from Teen Quest)
Snacks if you want them!

This consent form gives permission to seek whatever medical attention is deemed necessary, and releases Union Presbyterian Church of any liability against personal losses of the above named student. I the undersigned have legal custody of the student/students named below, a minor, and have given our consent for him/her to attend Laurelville High School Retreat. I understand the inherent risks involved in related activities and hereby release the Church, its pastors, employees, agents, and volunteer workers from any and all liability for any injury, loss, or damage to person or property that may occur during the course of my student's/ students' involvement in this event. *
