Participant Waiver Form
Please fill out and print the Participant Waiver Form and bring it to the clinic. You do not have to fill out the payment section on the waiver form. See below for payment information.
Follow the instructions below to pay for the clinic.
  • Log in to e~Funds for Schools - https://payments.efundsforschools.com/v3/districts/56064/
  • Under the Welcome bar select GBS Volleyball B Camp - Middle.
  • Confirm the dollar amount you wish to pay ($100 default is for both sessions, $60 is for one session only).
  • Enter the participant's name and a guardian contact email address.
  • Click the Add to Cart button.
  • Select Checkout to complete your payment and continue as Guest.
    • Online payments can only be made via a credit card or debit card. 
    • Both payment types attract a non refundable surcharge.
