I understand and am aware of the risk of COVID-19. I give my child(ren) permission to attend Sandlot Camp Westerville knowing risk is involved. I also agree to monitor my child(ren)'s temperature each day, to keep them home if it is 100.0℉ or greater, and to notify the Camp Director immediately if this occurs. I also understand that it is my responsibility to keep my child(ren) away from anyone who is +65-years-old and/or those with underlying medical conditions during my child(ren)’s week(s) of camp, and for 14-days thereafter. *
