Tobacco Workgroup Application
Please complete the application below if you wish to participate in the Fulton County Youth Tobacco Workgroup.  The focus of this group will be to prevent tobacco use, promote responsible use and conservation of the environment and strengthen environmental literacy among youth by educating, engaging and inspiring them to act on issues surrounding the impacts of tobacco use on the environment.  Membership in this group is voluntary and is open to any high school student who lives, works or plays in Fulton County.
Members will commit to attending monthly meetings to plan and implement a project focused on the Environment.  Members will receive a quarterly stipend for participation in the work of the group. All members will be required to complete a w-9 form in order to receive payment.

Representation of the Fulton County Tobacco Workgroup (FCTW) in schools, the community and through social media:  As members of FCTW, our students represent FCTW in their schools, in their communities as well as in the broader online community.  We believe FCTW members’ online as well as “in person” activities should not support, through actual behavior or interpretation, the use of substances, including tobacco, alcohol, marijuana, other illegal drugs or the misuse of over the counter or prescription medications.
