_____________________________CHORISTER PLEDGE:  __________________________  As a member of BAYC, I pledge that I will be an enthusiastic choir member demonstrating a positive attitude.  I understand that I am expected to attend all scheduled rehearsals and concerts.  I realize that I may have to work on my own at home practicing some music in order to learn and sometimes memorize the repertoire.  If I have to miss a rehearsal or concert I will give the Artistic Director as much advance notice as possible. MEDICAL TREATMENT AUTHORIZATION
I do hereby state that I have legal custody of the aforementioned minor. I understand that all efforts will be made to contact the parent(s) or guardians in the event of a medical emergency. If any injury or illness is life threatening, or in need of emergency treatment, I authorize the BAYC staff and volunteers to summon any and all professional emergency personnel to attend to, transport, and treat the minor. It is understood that this authorization is given in advance of any such medical treatment but is given to provide authority to the BAYC staff and volunteers in the exercise of his/her best judgment upon the advice of medical or emergency personnel. The BAYC, its staff and volunteers are not liable for any harm, medical or otherwise, from any disclosed or undisclosed medical conditions of its participants.

TRANSPORTATION
I agree to provide or arrange transportation for my chorister to ALL rehearsals and concerts, except in the event of serious conflict, in which case I shall notify the director as soon as possible before the scheduled event. BAYC may arrange for transportation to and/or between certain choir events. By signing below, I give permission for BAYC to transport my chorister as it deems necessary.

RELEASES AND INDEMNIFICATIONS
I understand that there are times when my chorister’s name, photograph, or likeness may appear in newspapers and on TV or Social Media in relation to his/her participation in BAYC. By signing below, I grant BAYC permission to use the chorister’s image, photograph, voice and features in this form of public domain, or on the internet for advertisement, public announcement, or any other purpose deemed fit by the organization. BAYC will not be responsible for the appearance of the chorister’s name in media over which the organization exercises no control. I agree to fully indemnify, release and hold harmless BAYC, its officers, directors, employees, agents, chaperones and volunteers (collectively “BAYC”) for monetary damages and from any and all liability and acts of negligence in connection with any injuries, damages, or harm which may be suffered by my chorister relating to any of BAYC’s rehearsals, performances, trips and all of its related activities. I understand that BAYC will provide adult supervision and/or chaperones at all rehearsals, performances, trips and related activities. If disciplinary measures are required beyond reasonable supervisory actions they will be handled by the Director or the Board Chair.  Nonetheless, I recognize and accept that accidents, including personal injury, can occur, despite all best efforts to avoid them.

I have read the above authorizations, terms, releases and indemnifications, fully understand them, and sign this Agreement, personally and on behalf of my chorister, as my own free act and deed.  (please sign below) *
-----------BAYC TUITION AGREEMENT and PAYMENT SCHEDULE------------
A 10% discount is offered for each additional younger sibling enrolled.

A separate agreement form needs to be filled out for each child singing with BAYC.

TUITION IS DUE WITHIN 2 WEEKS OF ENROLLMENT

As a non-profit organization, BAYC counts on tuition income. Continued enrollment in BAYC is contingent upon your tuition being paid in full as tuition is a vital part to the sustainability of our organization.  

The ultimate goal of BAYC is one of inclusion and education in the art of singing for all youth voices. In an effort to provide all children the opportunity to participate in the Bangor Area Youth Choirs, financial assistance may be available through the Michele Hall Voss and Jean Sigler Scholarship Fund. The Financial Assistance Form can be found on our Web Page under Registration.  Please fill out this form if you need Financial Assistance.

TUITIONS ARE NON-REFUNDABLE

Payment can be made two ways:

1.  PayPal through the "PAY TUITION/SPONSOR" button on the BAYC WEBSITE
2. Mailed to the address below:

Additional expenses for which parent(s)/guardian(s) are responsible include proper concert attire for the chorister enrolled in the BAYC.

● A fee of $10.00 will be assessed for each piece of music that is not returned.
● A fee of $20.00 will be assessed if the choral folder is not returned.

IN ORDER TO GET THE MOST OUT OF THE BAYC EXPERIENCE WE STRONGLY RECOMMEND ENROLLMENT IN THE FULL YEAR.  VOCAL AND CHORAL SINGING SKILLS ARE BUILT ON EDUCATION, PRACTICE AND EXPERIENCE AND WE WANT EVERY CHORISTER TO HAVE THE OPPORTUNITY TO TAKE FULL ADVANTAGE OF ALL WE HAVE TO OFFER.
