If you selected Cello, Violin, or Viola above, please indicate if you currently have a rented/owned instrument for home practice. **In order to have access to school owned instruments, families must have a rented/owned instrument for home practice.** *
If you selected bass above, please indicate if you would like to check out a second bass for home practice. *
*BOTH THE STUDENT AND PARENT/GUARDIAN should read through this agreement before submitting to ensure proper understanding of expectations. *
Instrument is distributed in working condition. Any issues with the instrument or damage must be brought to the attention of the director as soon as it is noticed. *
Students must not over tighten bows during class and MUST loosen the bow and carefully put away the instrument, bow, & rock stop/anchor at the end of every rehearsal. *
Students must only use director approved rosin brands & cleaning supplies and follow procedures on how to utilize these items properly. *
Issues relating to normal wear and tear will be repaired through the FPMTA budget. Any damage beyond normal wear and tear will be charged to the student's fee account. (some examples include dropping the instrument, dropping something onto the instrument or colliding with something that creates substantial dents/cracks to the body or breaks the bridge/neck/pegs) *
Students must maintain good classroom participation & behavior in order to use a school instrument. If the above expectations are not met, Fairview Park City Schools and the Mrs. Needham reserves the right to revoke the use of a school owned instrument and the student will have to provide his or her own personal instrument on a daily basis in order to continue. *
Once you submit this form, an instrument(s) will be assigned to your student. Students cannot use school instruments before this agreement is signed. *
