This form will be taken as an official entry into the Junior Nationals Team Competition 2024. Please submit an official entry form for each team you are entering.

Entries must be received by Friday 2nd September. 

Please complete all details of all players in their SquashLevels order as at August 30 grading list. All players entered will also be entered in the individual draw unless otherwise advised.
