We are in the process of preparing for a possible shift to Level 3. At Level 3, the key message is that if you can, you should keep your child at home.  You should only physically send your child to school if you need to.

This form is just for us to gather initial information to support with staffing.  This may change as more information becomes available from the Ministry of Health and Ministry of Education.

If your child has a health condition that means they are at a greater risk of a severe illness you must keep them at home.

If your child is sick please also ensure you keep them at home.

This form is for us to gain initial information regarding your intention during Level 3.  We will then need to work with Ministry Guidelines to develop a plan that sits within Health and Safety guidelines.  Early indications are that  these guidelines will be much stricter under the Delta variant.

These guidelines will most likely include:

- strict physical distancing
- strict hygiene protocols
- strict bubble conditions
- strict conditions around the use of equipment
