1. THIS FORM WILL BE UPDATED EACH WEEK TO REFLECT THE COMING SUNDAY SERVICE.
2. THIS FORM MUST BE FILLED BEFORE FRIDAY NOON EACH WEEK PRIOR TO VISITING NORTH EDMONTON ALLIANCE CHURCH FOR IN-PERSON SERVICE.
3. A REGISTRATION FORM MUST BE FILLED OUT EACH WEEK PRIOR TO THE SUNDAY AS SPACE IS LIMITED (maximum 50 persons).
4. AN EMAIL CONFIRMING YOUR IN-PERSON ATTENDANCE WILL BE SENT TO YOUR EMAIL.
5. IF YOU DO NOT RECIEVE THE CONFIRMATION FROM THE OFFICE, PLEASE DO NOT ATTEND IN-PERSON SERVICE. YOU WILL NOT BE ALLOWED TO ENTER.
6. THIS FORM MUST BE COMPELTED BY A PERSON 18 YEARS OF AGE OR OLDER.

- A mask must be worn at all times during the in-person service.
- Your temperature will be checked as you enter the church.
- Please use hand sanitizer provided as you enter and leave the church
- Please follow the arrows to help the flow of traffic
- The usher will take you to your seat.
- Please do not move to another section after you have been seated.
- Always wear your mask when you are in the church building.

- We kindly ask that you join our worship service through YouTube and refrain from attending in-person service until you are feeling better.
- You must isolate for 10 days from the onset of symptoms OR receive a negative COVID-19 test and feel better before attending in-person service.
- Use the AHS Online Assessment Tool or call Health Link at 811 to arrange for testing and to receive additional information on isolation.

We have limited space for attendance to keep social distancing within the church building, please note that not all registrations will be accepted due to seating limitations in the sanctuary. If you do not receive an email confirmation for the week you have registered, we invite you to worship with us on YouTube.

If you receive an email confirming your in-person attendance, but observe symptoms prior to Sunday, please contact the office immediately at 780-478-1443.
