This form should be completed in the event a swimmer intends to compete at an unsupported meet.  While no approval is required, as per the guidance provided by the club, the information will be shared with the Head Coaches, the Club Secretary and the club Welfare Officer to ensure the welfare and long term development of the child is ensured.
