How it works:

1. Please fill out this form in its entirety.
2. Go to each station to see sample sizes of the uniform items and choose the size that best fits your player.
3. Please choose three different jersey numbers in order of preference. We will do our best to give everyone their first choice, but current players will be given preference for their 2020/21 number.
4. Confirm spelling of last name for back of jersey.
5. You will receive an email confirmation to the email provided.
6. Double-check all entries for accuracy and confirm your choices as any changes/reorders will be at your expense. Please double-check!
