In the event of an injury or illness I/we grant permission to any and all health care providers designated by St. Leonard's Athletic Association to provide my/our child any and all necessary medical care related to the injury or illness. I/we further understand I/we will be contacted as soon as practical as to the medical emergency and be provided with all the necessary information related to the medical emergency.

I give my permission for my son/daughter to practice, compete and represent the St. Leonard's parish in St. Leonard's Athletic Association (SLAA) approved interscholastic activities.

I agree to fulfill my parental/guardian responsibility as part of my child's participation in the SLAA program by serving concessions as scheduled during regular season and tournament games or meets.

I understand that children enrolled at St. Leonard's must maintain academic standards for participation in extracurricular activities as outlined in their school handbook. Religious education students must satisfy enrollment and attendance guidelines of their parish's religious education program both in 2019/20 and 2020/21 school years. In addition they must maintain academic standards for participation in extracurricular activities as outlined in their own schools policies and procedures. As the parent/guardian of the participant in the SLAA athletic program I will notify the SLAA board of any change in my child's eligibility status.

I must furnish proof of a physical exam, including any physician recommendation restrictions, prior to my child participating in any SLAA athletic programs. Pupil accident insurance is required for participation per Milwaukee Archdiocese rules and such insurance will be maintained by me which will cover my child in case of personal injury.

I understand that promotional pictures (individual and group) may be taken during these sporting events. I give permission for my son's/daughter's picture to be used for promotional materials, such as the athletics web site, etc.

It is expressly understood and agreed that I will not hold SLAA, St. Leonard School, St. Leonard Parish, SLAA board members; coaches or any supervising volunteers liable in the event of an injury by my child in connection with their participation in SLAA approved interscholastic sports. It is further expressly understood that this waiver of liability on the part of the student named below or entities is a condition precedent to my child's eligibility to participate.

Failure to comply with the above conditions may result in my child not being allowed to practice or participate in games/meets. I have read the above statement and agree to all conditions.
