Please complete this form to request financial assistance for MYSA programs. Please note that a form will need to be completed for each sport season and each participant you are seeking assistance for.

In most cases MYSA will provide individual payment plans for participants, since our organization runs solely off of registration fees it is difficult for us to provide discounts and partial financial assistance.

MYSA does not provide full (100%) financial assistance for any program, we may provide assistance up to 75% off of registration fees or payment plans.

MYSA Financial Assistance Program

MYSA will not offer complete 100% financial assistance, we will offer discounts depending on individual situations of up to 75% off registration fees or provide the option of a payment plan that is set up on a case by case basis. MYSA does not provide financial assistance for extra items needed to participate, we can only provide assistance for registration fees.
