Book Fair Opt Out Request
Our school Book Fair is intended to promote a love of reading and to raise funds for purchasing materials for our school library. If you choose to have your student NOT participate in the Book Fair, please complete the form below. This Google form will help us generate the list of students whose parents or guardians have requested that their child not participate. 

Reminder: participation in the Book Fair is always voluntary. If you choose to opt your child out of the Book Fair, please help us help you in enforcing that request by talking to your child about not participating prior to arriving at school. 

If you have more than one child enrolled at this school that you want to opt out, please complete a separate form for each child.
