A central requirement of the schools Medical Policy is for an individual healthcare plan to be prepared, setting out what support each pupil needs. Individual healthcare plans are developed in partnership between the school, parents and any relevant healthcare professional who can advise on your child's case. The plan expands on the action we need to take should an emergency arise relating to your child's medical condition.

Please complete the plan below as soon as possible. If you require any assistance in developing your child's healthcare plan please contact us and we can make an appointment for you to meet with our Learning Mentor, Mrs Holmes.
