Thank you for taking this necessary step for volunteering as a CCDS chaperone or driver. PLEASE NOTE:  YOU MUST GET CLEARANCE THRU THE DOJ WITH CCDS AS THE REPORTING AGENCY.  Having your Live Scan done for another agency does not fulfill this requirement. The Dept. of Justice does not "share" clearances. To complete the Live Scan requirement, first print out the following document and take it to a local Live Scan agency. There are no Live Scan documents that you need to upload or submit to CCDS once you receive your Live Scan. We do, however, need to be notified that you have received services, so we may follow up with the proper agency. Please use this form to let us know where and when you received Live Scan services. We will notify you when we see you are cleared by the Department of Justice. Please be aware, this can take between 2-10 days, so be sure to allow plenty of time to get this process completed before the upcoming field trip.  

Please note, you only need to submit Live Scan clearance for CCDS once - this clearance does not need to be renewed. If you have volunteered for CCDS in the past and believe we already have Live Scan clearance on file, do not complete this form and contact the main office at CCDS.
