The Angel Tree program can provide clothes and toys for families who have students that receive free/reduced meals at school.  The Angel Tree Program is OPTIONAL, so please only fill out this form if you would like assistance at Christmas time.

Please complete the form below along with SHARING INFORMATION WITH OTHER PROGRAMS FORM, so that you will be contacted in October to give us your request for Christmas.  You MUST check the box on the Sharing Information with Other Programs form that you wish to be a part of the Angel Tree program.
