5th Grade Yearbook Questionnaire
As you are finishing up your time at Bethel Springs, the Yearbook Committee is excited to give you an opportunity to share your thoughts and memories in the 2019-2020 Yearbook. Please answer the following questions for inclusion in the yearbook; your responses will be featured next to your school/baby pictures.  

Please note that due to space constraints and for the sake of general consistency, your responses may need to be edited somewhat. We will try our best to preserve what you submit in its original form whenever possible.
