This form is designed for issues that require immediate review. If this is a general feedback of referees, please use the Feedback Form after the event to communicate these things.

An Emergency is a situation that needs the Head Referee to be alerted.
The NCHC will NOT remove a referee from a court they are scheduled on to accomadate a request or demand.
The NCHC does not remove referees or request a referee be removed during a game.
This form can not be used to attempt to move a referee.

The goal of this form is for leaders to be able to communicate with the NCHC about a situation that requires review from the NCHC staff and head referee.

The need for this form is 80% of the time that a feedback form is submitted during the event about referees it does not have the needed information to be able to follow up with the situation. This form is designed to gather all the imperative information to be able to hold the head referee and referees on site accountable, when needed.
