ONE CHILD: I agree to have $120 per month for 10 months charged to my FACTS Tuition Management account beginning August 20, 2022, and the last payment will be on May 20, 2023.
ONE CHILD: I will pay the full amount of $1,200 directly to the Business Office by the end of the business day August 12, 2022. If payment is not received, your FACTS Tuition Management account will automatically be charged (see above).
TWO CHILDREN: I agree to have $240 per month for 10 months charged to my FACTS Tuition Management account beginning August 20, 2022, and the last payment will be on May 20, 2023.
TWO CHILDREN: I will pay the full amount of $2,400 directly to the Business Office by the end of the business day August 12, 2022. If payment is not received, your FACTS Tuition Management account will automatically be charged (see above).

Withdrawal requests from transportation services must be submitted by the parent/guardian to North Hills in writing (email is acceptable sent to
school@nhcsvallejo.org
) no later than the 15th of the month to avoid additional charges.

I agree to provide written notice for withdrawal. Transportation is a month-to-month charge. There are no refunds for a partial month's use.
