Any current student wishing to remain in an FPCHS or MHS programs of choice must complete this application.
A Program of Choice provides an opportunity for a group of students to enroll in a specific set of courses with a designated focus.
Transfers are granted through completion of the highest grade at the school.
IEP requirements cannot be guaranteed for students accepting a transfer through Program of Choice.
If a student attends a program of choice that is not at their home zoned school, they are responsible for their own transportation.
Program of choice students are subject to school choice revocation based on grades, attendance, and behavior. If a program of choice student leaves the program, they must return to their home zoned school.
Please submit a separate application for each student.
