This is a online registration form

FILLING THE FORM FROM HOME:

1. Please fill the form if your booking is already confirmed and accepted. If you have a voucher or gift certificate, you will need to insert the voucher number below.
2. After you click sumbit, we automatically get your filled out form. If you filled out your form a couple of days before your arrival/ activity, you have done your part. On the day of the Activity, when you arrive at the airport just report youreself with your name at our manifest and tell that you have already filled out registration form.
3. We will find your form, print it out and the only thing for you to do is to review the form and sign it. After that the manifest will give you further instructions.

FILLING THE FORM AT THE AIRPORT:

1. Go through the questions and fill them out. You will need to insert the voucher Nr. if you have one.
2. After you complete all the questions and hit submit, we will automatically get your filled out form. Go to the manifest and get the print out version.
3. Review and sign the form and bring it back to the manifest. After that you are good to go, manifest will give you further instructions.

*Sometimes it takes up to 10 min for us to recieve the completed form, so please stay patient*

MEDICAL STATEMENTS
Below are several medical statements which you need to accept in order to meet our Medical regulations

I hereby declare that In the past 12 months I have not had any accidents which would cause bone fractures, torn ligaments, concussions or brain injuries, etc.

I am not suffering from any cardiovascular diseases, spine injuries, high or low blood pressure, anemia, diabetes, internal organ diseases, muscle disorders, etc.

MEDICAL STATEMENT 3 *
I have no experience with disturbances in consciousness, vertigo, respiratory, sinus or middle ear infections, etc.

MEDICAL STATEMENT 4 *
In the past 12 months I have not had or been treated for any mental disorders, including drug or alcohol addiction.

in the past 24 hours I have not drank any alcohol, taken any drugs or pills which would effect psychomotor skills.

RISK ASUMPTIONS
Below are several Risk assumptions which you need to accept in order to meet our Legal regulations
ASSUMPTION OF RISK 1 *
By signing this waiver I am applying for tandem skydiving and I also hereby release and discharge Skydive Bovec from any and all liability, claims, demands or causes of action that I may hereafter have for injuries or damages arising out of my participation in parachuting activities.

ASSUMPTION OF RISK 2 *
By signing this waiver I take all responsibility for any material damages to my property, especially for items like: specticals, jewelry, etc., which I haven't removed before the jump.I agree that I will not sue or make claim against Skydive Bovec for damages or other losses sustained as a result of my participation in parachuting activities.

ASSUMPTION OF RISK 3 *
I am aware that parachute instruction and jumping are hazardous activities, and I am voluntarily participating in these activities with the knowledge of the dangers involved and hereby agree to accept any and all risk of personal injury, death, or property damage.

ASSUMPTION OF RISK 4 *
I hereby agree that the jumpmaster, all employees, representatives, organizers or any other person connected with Skydive Bovec, such as aircraft and airport operators or any parachuting organizations, will not be held responsible for any injuries or other issues that may occur to me, my heirs or next of kin during the tandem free fall and parachuting, as well as during the plane ride.

ASSUMPTION OF RISK 5 *
I understand that I have no additional insurance for the tandem parachuting and plane ride and my insurance company may offer a lower compensation in case of any injuries. If the tandem passenger wishes, they can be informed of the insurance rate for passengers in case of an accident during the plane ride and during the jump.

ASSUMPTION OF RISK 6 *
I hereby agree that I will act accordingly and follow all instructions made by the jumpmaster and the aircraft pilot at the aircraft runway, in the vicinity of the aircraft and during the jump. I hereby agree that I will approach and enter the aircraft only in the presence of my jumpmaster or other accredited personnel, and only after all the instructions for the jump and plane ride have been explained to me (all the proceedings inside the aircraft, body position for the jump, during the free fall and for the landing).

I understand that my tandem jump will/may be recorded and agree that any photos, video or audio material can be published commercially in print or via electronic media. I understand that the recording is intelectual property of Skydive Bovec and will not use it without their permission for any commercial or advertising use.
