COVID-19 LIVE Grant Application
Under this initiative community-based coalitions can apply to their local United Ways for flexible funding and additional vaccine allocations in order to bring vaccinations to the gathering places and even the doorsteps of minority and vulnerable populations (African Americans, Latinos, immigrants, refugees, people who are elderly and isolated, people experiencing homelessness, urban and rural populations living in poverty, single parents, etc.) in areas of disparity and need across the state (excluding Philadelphia county).
Municipal entities, community-based organizations, and local nonprofit health centers are invited to submit innovation plans to request funding and, if needed, additional vaccine. For example, a local Meals on Wheels might partner with their FQHC to get vaccines to seniors’ homes, a local pharmacy might partner with an African American church to bring vaccines to their location after Sunday services, or a local food pantry might partner with their hospital system and a bilingual, Latino-focused nonprofit to vaccinate people when they pick up their food in their neighborhood.
All grant activities must occur within 60 days of invoicing, which occurs at the end of each month.
Funding Range:
Minimum impact would be 20 people vaccinated for a $400 grant. Maximum would be 2,000 people vaccinated for a $40,000 grant.
