Hicksville High School Transcript Request Form for Mrs. Felber
Upon receipt of this request and all required materials, the Hicksville High School Guidance office will electronically submit an official High School Transcript and supporting documents to the school(s) listed below. There is a processing period of 15 school days from the date of receipt of this form and submission of all required guidance materials. Fill one form out for EACH transcript you need sent.
Student Responsibilities:
Students are responsible for requesting teacher letters of recommendation., and send any SAT and/or ACT scores directly from the testing center to the college(s) to which they have applied. Please make sure that the schools listed below are on your "Colleges I'm Applying To” list in Naviance/Family Connection and that you have filled out the FERPA form on Naviance. Transcripts CANNOT be sent out until the FERPA form is signed by you.
THIS FORM IS REQUIRED TO RELEASE INFORMATION TO THE SCHOOLS LISTED BELOW.
GUIDANCE MATERIALS WILL BE SENT ONLY TO THE SCHOOLS ON THIS FORM
