請於108年 10月16日(星期三)下午6點前繳交以下資料至行政大樓4樓國際事務處Jasmine。
書面繳交資料如下(請依照順序排序繳交一份):

(1) 杜拜阿聯酋餐旅學院(EAHM)學校申請表(2吋彩色照片一張，黏貼在杜拜學校申請表格右上角)
    (請連結至網頁下方下載pdf檔,此申請表務必請用手寫，請勿用電腦打字)
(2) Medical information form此為非正式體檢表，按照身體過去狀況初步自行檢測填表即可
   (請連結至網頁下方下載附件)
(3) 一年級上下學期英文版歷年成績單
   (含班排名，註冊組申請正式版本，同學可先至行政大樓一樓飲料販賣機旁邊的小額付款機先付款，再
    至註冊組申請)
(4) 英文履歷表一份 (不限格式跟頁數)
(5) 語言證照成績影本 (若面試前未達門檻或未能於面試當天提供相關語言證明者，此項學生可於面試之
    後後補，繳件截止時間會另外公告，逾期繳交視同放棄)
