Parent Consent Form for Visits Around Discovery Bay
Dear Parents

We are very excited about using our natural environment in Discovery Bay to enhance the students’ learning. There are many areas of Discovery Bay that we would like to explore with the students. From time to time we need to take the students to some of these areas to facilitate their inquiries. We see our local environment as a primary learning resource. We are committed to giving the students opportunities to do primary research in their local area environment.

As an example we have conducted an inquiry into Biodiversity and have used the Mangrove Swamp as a case study. Other areas that could be of interest may include the beach area, Siena Park and the numerous walking tracks, which make Discovery Bay such a special environment.

The staff will compile a series of risk assessments for all places that we intend to take the students. These assessments will ensure that all the trips are safe and enjoyable for all. From time to time we will take the students on short walking expeditions to investigate particular topics in their environment.

We ask that you complete the attached permission note so that we can do this and return it to Discovery College.

I understand that, whilst the staff in charge of the members of the group will take all reasonable care for their health and safety, they cannot be held responsible, unless they are found to be negligent, for any injury, illness, damage or loss suffered by my son/daughter during or arising out of the journey. I authorise the leader of the trip or any member of the school staff present, to consent to such medical treatment - including inoculations, surgery, blood transfusions - which, in the opinion of a qualified medical practitioner, may be necessary for my child in the course of the trip. I, therefore, agree to indemnify the English Schools Foundation, its employees and agents against all liability for injury (including death), illness, loss to person or persons or damage of property caused by my son/daughter unless this can be shown to be due to the negligence of the English Schools Foundation or any of its employees.
