Maine Stars Registration Form  22/23        
Please fill out all portions of this form ( one per child) for the 2022/2023 cheer season. This form must be submitted prior to your child trying out or participating in any tumble class or clinic. THIS A TUMBLING ONLY REGISTRATION FORM.  Please be advised that due to the ongoing COVID- 19 pandemic our class sizes may be limited in some cases.    Please visit our website www.mainestarscheer.com and click the forms tab.  The information under this tab explains what we offer and our protocols in response to the pandemic should it come around again.   Also, all payments will need to now be paid via PayPal.  You can find that link on the website on the homepage...it's the yellow "Buy Now" button.  Please assist me by writing in the memo line your child's first name and what the payment is for.  For example - Susie Smith - Registration Fee.   A one time registration fee for this new season is $60 for tumbling, competitive cheer including prep, and fundamentals cheer .  Season 22 begins at Maine Stars....Come join us...We LOVE what we DO and WILL LOVE working with YOU!

Consent and indemnification of risks due to participating in all activities at Maine Stars throughout the cheer season which runs from MAY 21, 2022 - May 31, 2023
Due to Covid-19 and the new normal we presently find ourselves in, your signature indicates you are fully aware of the risks associated with ALL ASPECTS OF CHEER and TUMBLING and agree to hold harmless, Maine Stars, it's owner, staff, or any individual who instructs the athletes should an injury OR SICKNESS occur.  Furthermore, due to the present pandemic you are entering Maine Stars at your own risk understanding, there is no guarantee that the virus will not be contracted by your child, or furthermore passed on to other family member due to your child's participation in any and ALL events throughout the cheer season.  Maine Stars will attempt to minimize the risk of infection (PER CDC GUIDELINES) by setting up social distancing, limiting class sizes, sanitizing surfaces, mats, and the bathrooms, and limiting social gathering IF the state of Maine AGAIN enforces mandates regarding the virus.  When it is safe to lift these precautions you will be notified. However, Maine Stars is not responsible for cheer/tumbling injuries or the spread of any virus or any contagion, and you as the parent/guardian of the above-named child have made with confidence a personal decision to allow your child to participate, have assumed and accepted the possibility of injury both minor or major, which includes death and hereby consent that your child may participate without restrictions for the 2022/2023 cheer season.  Your signature also makes you responsible FOR ALL FEES  associated with the program and you understand that all fees are nonrefundable regardless of the reason why you choose to leave the program.
