Enthusiastic Sobriety Anonymous Mass Complaint Form
Complaint Purpose

The aim of this complaint is to shed light on the practices and treatment within Meehan-affiliated Enthusiastic Sobriety Programs. Our goal is to ensure that the voices of both past staff and client members are heard and taken seriously. The collected data from this survey will primarily be used to inform state licensing boards, governmental oversight agencies, and survivor advocacy groups about the issues within these programs. Additionally, we encourage survivors, especially those who have left the programs within the last five years, to file individual complaints. Support is available for anyone who needs assistance during the complaint process, regardless of when their experience occurred.

To further raise public awareness, this anonymized data will be shared with media organizations. Rest assured, no identifying information will be disclosed without your explicit consent.

Risks and Safeguards

Your answers will be completely anonymous. None of your identifying information will be shared without your explicit permission.

The results will be presented as aggregated data, and specific incidents will be described only to illustrate broader trends without revealing personal details. Should the need arise to release complaint findings, all personal identifiers will be carefully redacted.

The only piece of identifying information that is required is your email. This is so that the Research Team can contact you if needed.

Please be aware that discussing or recalling past experiences can be emotionally challenging. If you feel overwhelmed at any point, you are encouraged to pause your participation and seek support if necessary. Contact information for support services is provided at the end of this survey and upon submission of your complaint.

Procedure

You will be asked to complete various questions about yourself and your experiences within any Enthusiastic Sobriety program. Please also include your time on Staff, if applicable.

The survey is structured to capture a comprehensive understanding of your journey and its long-term impacts. It takes about 20-40 minutes to complete, and progress will not be saved until you hit "Submit." To ensure no data is lost, please keep the browser open during breaks.

Consent Statement

By participating in this survey, you consent to the use of your non-identifying information to compile general complaints against Enthusiastic Sobriety programs. This includes the dissemination of such information through media channels to enhance public understanding and spread awareness. Your participation is voluntary, and your identity will not be publicly linked to any information gathered during this process.

Specifically, you agree to:
  • Allow your non-identifying information to be used in a general complaint against all Enthusiastic Sobriety programs.
  • Allow this information to be used by media outlets to spread awareness to the general public.
  • Understand that all information provided will be stored responsibly, and non-identifying information may be compiled for further research by the Enthusiastic Sobriety Abuse Alliance.
  • Allow your answers to long-answer questions to be used as quotations to substantiate complaints or explain the circumstances of events discussed in the complaint form
As a participant in this survey, you have the following rights:
  • You have the right to stop participating in the survey at any time, for any reason, without needing to provide an explanation.
  • You have the right to refuse to answer any or all questions during the survey.
  • You may choose to provide as much or as little information as you feel comfortable sharing.
  • You have the right to withdraw from the survey at any point, effectively discontinuing your involvement.
