What type of booth will you be setting up? (Setup space limited to 10'x10', multiple space can be purchased) *
We will do our best to accommodate all vendors, we will also try to avoid duplication of products being sold- first come, first serve. By signing below, you agree to pay the $30 vendor fee + 15% of proceeds to be donated to MYF. 

In consideration of the acceptance of participants to Martinez Youth Football & Cheer (MYF) and MIDNIGHTERS Car Show event by execution of this entry form. I/We hereby release MYF, MIDNIGHTERS, Martinez School District (and Alhambra High School), and their members, officers, and anyone else connected with this event from any known and unknown damages, injuries, losses, judgments, and/or claims from any causes whatsoever that be suffered by anyone participating in this event. I/We accept responsibility for Bank fees on returned checks.
