Show event by execution of this entry form. I/We hereby release MYF, MIDNIGHTERS, Martinez School District (and
Alhambra High School), and their members, officers, and anyone else connected with this event from any known and
unknown damages, injuries, losses, judgments, and/or claims from any causes whatsoever that be suffered by anyone
participating in this event. I/We accept responsibility
for Bank fees on returned checks.
