Please complete the information below if your child is planning on participating/ trying  out for the in the fall season for girls volleyball*, boys soccer* or co-ed cross country**.  All sports are open  for all grades students to try out/ participate.

* try outs are requiered for roster spots for boys soccer and and girls volleyball
** there are no try outs required for cross country.

Welcome to the Smart Fall sports season.

By completing the information below, I am giving my child permission to participate in the try outs/ practices selected at Smart . 

As a parent/ guardian I understand that practice is from 3:00-4:30 approximately and will pick my child up no later than that time.  Please see the attached hand out at the top of this form for coaches information and information about the team. 

I understand that a physical is required to participate starting on the first day AND have turned it in to  the office already or will turn it in on the first day of practice August 29th.
